Reverend Cotton Marcus, a preacher from Baton Rouge, Louisiana, agrees to star in a documentary about his life. He reveals that his faith has been shaken since his wife's difficult delivery and he's skeptical of exorcisms, which he believes are fraudulent but can be effective for believers. Cotton takes the documentary crew to perform an exorcism on Louis Sweetzer's daughter Nell, but their reception is hostile, and they soon uncover the dysfunctional Sweetzer family's deep-seated issues.
