The movie is a sequel to the Jean-Claude Van Damme film, focusing on his character's brother, David Sloan, a karate gym owner. After Tong Po, the brutal kickboxer, kills Kurt Sloan, the Thai government official seeks revenge by having Tong Po face David instead. When David is injured, his teacher, Xian, is brought in to help him recover, and they eventually set him up to fight Tong Po, leaving the question of who will survive.

In the 1990 film Kickboxer 2, David's brother Kurt inflicts a humiliating defeat on Po in the 1989 film Kickboxer. Seeking vengeance, Po brutally murders Kurt, then manipulates David into agreeing to a rematch.
