Dr. William Fredrickson, a seasoned psychotherapist, reflects on the most impactful patient of his nearly three-decade career: Charles, a man suffering from schizophrenia. Despite years of treatment, Charles's condition worsens, and he retreats to his family's cabin, where he's haunted by voices and forced to commit murders by the Grim Reaper, who threatens to take his own life if he doesn't comply. As the murder rate increases, Dr. Fredrickson becomes convinced that Charles is responsible and enlists the help of a young patient, Matthew, who suffers from manic depression, to find him.
